DMCA APPLICATION

In order to ensure the best possible operating environment for students and faculty the DMCA staff asks that you please fill out the form below and submit it before using the facilities. All applications will be processed promptly and you will receive an e-mail or phone message in response to your application within 72 hours (3 working days). Oftentimes applications will be processed the day of their submission. Priority will be given to faculty and students currently enrolled in classes being taught at the DMCA and graduate students in member departments.
