How do I get a badge?
First, attend a Radiation Safety Orientation Seminar. Once you are trained, complete a "Request for Radiation Monitor" form (click here to download form). Page 18 of the Radiation Safety Manual also has a form which may be copied and completed. Return the completed form to Radiation Safety. You can be issued a badge immediately or the badge can be forwarded to you through Campus Mail.
