Research Materials Shipping Forms and Labels

All packages of hazardous materials must be marked/labeled with the following information:

  • Name, address and contact information of the shipper (in addition to the information provided on the airway bill).
  • Name, address and contact information of the recipient (in addition to the information provided on the airway bill).
  • Proper shipping name for the hazard.

Packages containing dry ice: Biological Substance, Category B material; or infectious Substance, Category A material also require the applicable hazard label(s).  For your convenience these labels are provided below.

You may use the checklists below to ensure that you pacage is marked/labeled properly and that all required information is present on the airway bill.

Shipper's Declaration Form for Black and White Printers - FedEx:
This form requires the use of special FedEx form blanks, called LZR DG DEC.  These form blanks contain preprinted red vertical hatchings.  When these templates are used, they will print the Shipper's Declaration within the red hatchings.  Customers in the U.S. can order LZR DG DEC blanks part #157295 by calling 1.800.GoFedEx (1-800-463-3339).
