Coverage begins for Medical
If your date of employment is the first day of the month, your medical coverage will be effective on the first day of the month. If your date of employment is after the first day of a given month, your medical coverage will be effective on the first day of the month following your date of employment.
Coverage begins for Dental
As a service & maintenance employee, dental coverage is automatic for you and begins on the 1st of the month following your date of employment. Spouse and dependents may be added to your dental coverage with coverage being effective on the 1st of the month following your date of coverage.
Coverage ends for Medical/dental
Your medical or dental coverage ends on the last day of the month following your termination date.
Example: Employment ends May 15th, coverage ends June 30th.
Enrollment applications for medical and dental must be received by the Benefits Office within 30 days from the date of employment or status change.
