Medical & Dental Plan Rates
Medical Plan Rates (excluding any approved subsidy)
Medical plan rates are available on the Health Care Subsidy Form. As a post doctoral fellow, you can apply for a health care subsidy through the University by completing that form. Unless you apply for a health care subsidy, you will pay for the total cost for your medical coverage. You must submit the form to your department for approval within 10 days after your appointment begins, and your department must approve the form and fax it to Employee Services at 203-432-5153 by the 15th of the month in which your appointment begins.
