Death in the family
If your spouse or dependent child has deceased, this is a qualifying life event that allows you to disenroll your deceased family member from Yale's medical or dental providers.
Effective cancellation date will be the date of their death.
You must add the death of your spouse or dependent child as a qualifying life event within 30 days of the date of the death using the My Benefits website, which can be accessed through the University portal (www.yale.edu/portal). For instructions, please refer to the My Benefits: Adding a Life Event Quick Guide.
The following documentation is also required and must be submitted to Hewitt:
