Give birth, adopt, receive legal guardianship
If you have a newborn child(ren), have adopted or became the legal guardian of a child(ren), this qualifies as a life event and allows you to enroll your newborn, adopted child(ren) to Yale's medical or dental providers. You must notify the Benefits Office within 30 days from the date of the qualifying event.
The following documentation is required:
- Legal adoption agreement
- Court appointed legal guardianship agreement
You must complete an Enrollment/Change form, as well as a Benefits Revision Request form and submit along with your supporting documentation to the Benefits Office at 155 Whitney Ave, Room 130 or via fax at (203) 432-7575.
